The alteration package is stale: the policy's product module definition changed, the policy's values for what the package contains changed (see is_stale), or re-quoting the package against the current policy gives a different premium, sum assured or billing frequency. Refresh the alteration package and apply the refreshed one. Only returned when your organization has alteration package refresh enabled; if the re-quote itself fails, apply returns that error and changes nothing.
